Diabetes Self-Management Education Program

Perry County Memorial Hospital has a Diabetes Self-Management Education Program approved by the American Diabetes Association and staffed by a certified diabetes educator and dietician. A physician order is needed to participate. To schedule an appointment, contact Outpatient scheduling at (812) 547-0145. Most appointments are scheduled on Wednesdays, but arrangements can be made for other days.

The Burden of Diabetes in Indiana

Information from the Indiana Department of Health

  • 9.6% of adults, or about 455,000 in Indiana have diabetes. An estimated 250,000 adults have undiagnosed diabetes.
  • Diabetes is the 7th leading cause of death for Indiana residents.
  • Adults with diabetes were more likely than adults without diabetes to report ever being diagnosed with a heart attack or stroke.
  • Adult diabetes prevalence increased from 2004-2008 in Indiana.
