Every three years, Perry County Memorial Hospital conducts a Community Health Needs Assessment (CHNA) — and it’s more than just a requirement. It’s our opportunity to hear directly from you, the people we serve.

What is the CHNA?

The Community Health Needs Assessment is a collaborative process designed to identify the most pressing health needs in Perry County. It helps us understand the challenges residents face, the barriers to care they experience, and the services or programs that could make a difference. By gathering feedback from community members, local organizations, and healthcare providers, we are able to develop an informed action plan that addresses gaps in care and improves the overall health of our population.

The CHNA is essential to shaping how Perry County Memorial Hospital — and other local health partners — respond to the needs of the community. Whether it's expanding services, launching health education initiatives, or improving access to care, your input directly influences what comes next.
