We believe in giving back to our community.
Perry County Memorial Hospital proudly sponsors many community events and
works with
the community to provide health and wellness activities
Funds are limited, and the hospital is unable to approve all requests.
To be considered, a
request for sponsorship or funding should meet the following criteria:
- The event or project must directly benefit the geographic area served by Perry County Memorial Hospital.

The event or project must relate to one or more of the following community
health priorities:
- Health, Fitness, Wellness and Health Promotion
- Recognized Community Health Issues
- Disease Prevention
- Access to Health Care Services
- The event or project must be consistent with Perry County Memorial Hospital’s mission, vision, values and goals.
How to Apply for Funding:
- Review the application guidelines
- Download the Community Event/Sponsorship Request Form (enter new form link attached here)
- Complete the application and submit it along with all supporting materials and documentation.
