Home Care Services

Our Home Care team won the 2023 Home Health Honors Elite Award from Healthcare First, for going above and beyond in providing quality care and positive experiences for our patients.

Perry County Memorial Hospital offers a hospital-based home health program that provides a smooth transition from hospital to home with the assurance of on-going, quality care. It’s a team effort involving the patient, doctor, family and our Home Health caregivers and is available part-time to all homebound patients including the disabled, chronically ill and those recovering from surgery. Home Health caregivers are on-call 24 hours a day.

Nursing: A registered nurse provides professional nursing care and treatments, develops a physician- authorized plan of care and coordinates other services that are being provided by Home Health.

Home Health Aides: Individuals trained to work under the supervision of registered nurses to help with daily routines such as limited household tasks and prescribed exercises.

Physical Therapists: If you are unable to leave your home due to illness, injury or surgery, a physical therapist can provide home care. Physical therapists are licensed healthcare professionals who play a primary role in the rehabilitation of patients in need. Their expertise lies in the evaluation of physical dysfunction and the development of treatment plans to correct and increase functional independence.

Occupational Therapists: The primary role of occupational therapist is to assist patients in maximizing their independence by improving their ability to perform activities of daily living, such as grooming, bathing, eating, toileting, and dressing. Limitations in performing these skills can be due to illness, injury, weakness, or impairment of cognitive skills (e.g., problem solving, memory, judgment, and reasoning). In addition to instructing patients in exercises to improve function and reasoning, occupational therapists also teach energy conservation and efficiency techniques in performing these tasks.

Speech Therapists: Speech, language, and swallowing disorders stem from a variety of causes and affect people of all ages. These causes can include strokes, brain injury, Parkinson’s and Alzheimer’s diseases, and cancers. Speech-language pathologists work with patients to assess and treat speech, language, and swallowing difficulties in their home.

Medical Social Services: A licensed social worker is available to assist patients and their families with adjustment and problems related to their diagnosis. This individual will work on resolving problems that may add stress and slow down the recovery process.

To Qualify:

Home Health Care Services are available to patients who meet the following qualifications:

  • A doctor must order specific, skilled home health services.
  • A patient, and possibly the family, must be willing and capable of learning self-care skills, which will allow them to work with our home health team.
  • The home setting must meet the needs and safety of the patient, or be arranged to do so.
  • The patient must reside in either Perry County or Spencer County.
  • The patient must be homebound, but does not have to be bedfast.
