Perry County Memorial Hospital Receives American Diabetes Association Recognition

TELL CITY, INDIANA – Perry County Memorial Hospital received an on-site audit on May 9, 2011 from the American Diabetes Association in reference to our Diabetes Self-Management Program. The auditors reported to the Decision Subcommittee of the Education Program Recognition Committee on May 24, 2011, recommending that the program continue with recognition.

To promote quality education for people with diabetes, the American Diabetes Association (ADA) endorses the National Standards for Diabetes Self-Management Education. To support this goal, the ADA Education Recognition Program (ERP) assesses whether applicants meet the National Standards for Diabetes Self Management Education. The Standards are designed to be flexible enough to be applicable in any health care setting, from physicians’ offices and HMOs to clinics and hospitals. The ADA ERP is one of three and the most experienced Centers for Medicare and Medicaid Services (CMS) deemed certifying bodies for Diabetes Self Management Training.

Perry County Memorial Hospital has a Diabetes Self Management Education Program approved by the American Diabetes Association and staffed by a certified diabetes educator and dietician. A physician order is needed to participate. To schedule an appointment, contact Outpatient scheduling at (812) 547-0145 for an appointment. Most appointments are scheduled on Wednesdays, but arrangements can be made for other days.
